Waiver Form
To create a waiver of liability form for customers to sign upon booking a simulator, go to your settings menu where you have a forms feature.
On that forms page there is an option to create a new form.
Give the form a label, waiver of liability.
In the message field, enter all of your content for your waiver. Tell your customers everything that you want them to know in this message field.
At the end of the content, create a signature line.
To require that customers sign this signature line for your waiver, set the requirement to yes.
If set to no, customers can skip the signature line.
Next, make an assignment. Most waivers are assigned to the customer login, which means the customer will need to sign your waiver before they can perform any other action in your software.
There is a date range which will typically run for a long period of time. You can set this out to 2085 so that it will not expire any time soon.
There is a save multiple copies option. Usually with waivers there is only one signature, so this can be set to no. If set to yes, the customer will need to sign the waiver every time they log in.
There is an email option which if selected will send a copy of every form that gets filled out to your email.
Save the page when it is completed.
Once saved, that introduces the fields form.
This is where you can create a signature line so that your customers can sign your form.
Click the add new button to start that process.
Signatures can be hard to read, so you can create two fields for this where you have your customer name and a text box.
This will allow them to type their name in the form.
You might require that customers do not skip that field.
For a waiver leave registrants set to no.
You might also include some default text so that your customer can know what you expect in that field.
First and last names are usually desired here.
You can also add one more field that is going to be a digital signature.
The field name for that will simply be signature.
Require that customers do not skip the signature field. Set that to yes and save.

You will see the title of the form here.
Keep in mind in the preview you will not see your content, but it will appear on the real form below the label and above the first field at the bottom of the page. That will expand to hold all of your content and then your customer can type in their first and last names and provide a digital signature.
